ELIGIBILITY CRITERIA:  Mothers ages 15-21 years of age with a baby birth to 2years of age.  The mother must be in some form of educational program such as GED, Junior college, Etc.  (The home helps get the mother into the program of her choice.)  The baby must have daycare.  DHR must pay for the baby’s daycare through CMA.  Referral is made on application DHR-DFC 1870.  Babies First is under state contract and also receives board payments on mothers and babies placed there.

ELIGIBILITY CRITERIA:  Referral consisting of DHR 1870 and current social summary.  Serves male and female children and young adult’s ages 10-21 years of age.

SUMMARY OF SERVICES: Brantwood’s program currently provides care and supervision for up to 30 boys and girls.  Each child is given medical and dental care; enrolled in either academic or vocational programs in local schools, counseled by professionals, and encouraged to maintain family contact when appropriate.  Provides and independent living program to prepare older youth (ages 16-21) for living independently.  Brantwood offers respite care services to allow children living with foster families an opportunity to spend several days away from home each month.  Brantwood’s transitional living program provides off-campus services to teens that allow older adolescents the freedom to live on their own while still under the watchcare of Brantwood.  The program accepts male and female older adolescent youth as well as pregnant teen and teens with children.  Brantwood requires regular school attendance and for the teen to maintain employment.
